Copy of driving license if the Insured was driving the vehicle at the time of accident

Waiver Of Premium Claim

Note

  • Kotak Life Insurance must be informed of the insured’s death within one year of the occurrence of death.
  • The Accidental Disability Guardian Benefit, if availed, will cease automatically on the admittance of the Claim under the Life Guardian Benefit.
  • The Life Guardian Benefit Rider is subject to the conditions and the exclusions as laid down in the “Annexure LG” within the Policy Contract.
  • Copy of certified post-mortem report/ chemical analysis report (viscera) applicable for natural death only if post-mortem is conducted.
