Waiver Of Premium Claim

A. Waiver of premium in the case of death of the Policy holder (Life Guardian Benefit Rider)
For Waiver of Premium claims where the Policy holder has availed of the Life Guardian Benefit and the policy holder has passed away, future premiums as mentioned in the contract will be waived.

The following documents need to be submitted:

Note:
  • Kotak Life Insurance must be informed of the insured's death within one year of the occurrence of death.
  • The Accidental Disability Guardian Benefit, if availed, will cease automatically on the admittance of the Claim under the Life Guardian Benefit.
  • The Life Guardian Benefit Rider is subject to the conditions and the exclusions as laid down in the "Annexure LG" within the Policy Contract.

#Copy of certified post-mortem report/ chemical analysis report (viscera) applicable for natural death only if post-mortem is conducted

B. Waiver of premium in the case of disability of the Policy holder (Accidental Disability Guardian Rider)
For Waiver of Premium claims where the Policy holder has availed of the Accidental Disability Guardian Benefit and the policy holder has become totally and permanently disabled, the Basic Premium amount will be waived.

Life insured's copy of bank passbook / statement with Bank acount Details  
Disability certificate from the relevant government authority

Note:
  • The Life Insured will have to undergo the required Medical Tests by the Company specified Doctors.
  • The Company should be intimated with the relevant details:
    • Within 30 days of the accident and
    • Within 120 days after the happening of disability with proof.
  • The Life Guardian Benefit, if availed, will cease automatically on the admittance of the Claim under the Life Guardian Benefit.
  • The Rider is subject to the conditions and the exclusions as laid down in the "Annexure ADG" to the policy contract.

Additional Information:
  • All the documents submitted to us should be original or photocopies attested by a Gazetted Officer, SEM, Magistrate or a person of local standing, e.g. Sarpanch, Talathi, Tahsildar or Police Sub-Inspector.
  • All medical reports, documents and certification shall be issued by the attending physician and who is qualified to provide such document / certification according to the laws of India.
  • Depending on the facts and circumstances of the claim, the Company reserves the right to call for and request certain additional documents.
  • Kotak Life Insurance reserves the right to conduct and independent investigation / inquiry to be made into the bonafides of any claim where it is considered necessary.
